I'm imagining a device shaped like a shallow cup with four buttons on the bottom, oriented as top, left, right and bottom. This device would be locked over the users mouth like a gag, so that the buttons must be pressed with the tongue.
This device would act as a controller for software similar to the popular "Rock Star" titles. Colored lights would indicate when each button is to be pressed.
The standard mode could have levels of increasing difficulty. Perhaps a "speed mode" would simply require the user to press one button at more than "x" bpm for "y" minutes. Punishments would be issued when too many "notes" are missed in a set time, and the course would be failed if this happens to often during one lesson.
